CAMP 2011

Camp session:  June 14th – July 21nd, 2011 
Monday – Thursday; 8:30 am – 1:00 pm  (no camp on Friday)
Drop off:  8:15 – 8:30 am at Parkside Intermediate School, Westlake OH
Pick up:  1:00 pm at Patterson Pool, Clague Park

Tuition

Total tuition -  $1000 for the six-week session

Registration late fee of $100 for all registrations postmarked or completed online after May 20th, 2011. 

• Enrollment is limited based on space availability. 

• Full payment must be made on or before the first day; alternatively, by arrangement, weekly payments may be made each Monday.

• If your child’s tuition is being paid by other agencies, please download the ESY form below and submit by May 20th.

All 2011 Registration and Information Material will be submitted Online.

• Complete all required fields

• You will receive an automated response when your submission is successful.

• Your information will not be sold to or shared with any third party.

• If you cannot complete the Registration Online, please call or send us an email requesting a hard copy.

• A Late Registration Fee will be applied to the parent if these forms are received after May 20, 2011.

• Download the PDF located above or in the box to the right

• Complete with signature and return to LakeShore by mail (P.O. Box 45159 Westlake OH 44145), brought to the Parent Meeting, or turned in on the first day of Camp.

• We will not be able to accept your child at camp without this signed General Release Form.

 

Extended School Year Services

If your child will receive ESY services, your school district must submit the ESY Services Form and student IEP with clearly marked objectives by May 20, 2011.

• Please have the conversation with your school district EARLY regarding ESY.

• Your school district should have received this ESY Services Form, however if you would like to take it with your to your ESY meeting, it is available for download above or in the box to the right

• The IEP and highlighted objectives ensures that our teachers have the necessary information on each camper prior to the start of camp and can implement proper instruction on the first day.

• A Late Registration Fee will be applied to the school district if these forms are received after May 20, 2011.
